If you have a medical question during regular office hours, please call the Glades Road office at (561) 391-5515 or Clint Moore Road office at (561) 939-0800. Your questions will be answered as soon as possible.


Prescription refills must be requested weekdays only during the hours of 9 a.m. to 3 p.m. Refill requests cannot be accommodated outside of regular office hours.

Office Hours and Appointments

All appointments are scheduled in advance with the physician of your choice by calling the Glades Road office at (561) 391-5515 from 8:00am to 5:00pm, Monday through Friday; or the Clint Moore Road office at (561) 939-0800 from 9:00am to 5:00pm, Monday through Friday. Please call our office for additional information.

New patients are always welcomed.

Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule.

Everyone at our office is at your service.

Cancellation Policy

If you are unable to keep an appointment or need to reschedule, we ask that you kindly provide us with at least 24 hours notice. If not, a cancellation fee will be charged.


We are always able to give prompt attention to emergencies. During office hours, there is at least one doctor in the office attending to emergencies needing prompt treatment. One of our physicians or physician's assistants is also on call after regular office hours and weekends. To reach a physician after regular office hours, call the Glades Road office at (561) 391-5515 or our Clint Moore Road office at (561) 939-0800.

Your call will be answered by our answering service, and the physician or the physician's assistant on call will return your call as soon as possible.

Due to the unscheduled nature of emergencies, occasional delays in the appointment schedule do occur. We hope you understand that these delays are unavoidable, and that in an emergency you would be extended the same consideration.

Billing Inquiries

Our Billing Office staff with gladly assist you with any questions or concerns that you might have about your account.

You may contact the Billing Office: Monday through Friday, 8:00 am to 5:00 pm at (561) 300-1767.

Insurance Programs

As a service to you, our office staff will file your insurance claims when you have provided us with all the necessary information. We fully participate in the Medicare program and are members of most leading PPOs and HMOs as listed below. We are providers for many other commercial health plans, as well as for Workers Compensation, PIP and Auto insurances. Please contact our offices to determine if we are providers for your health plan. Please understand that we cannot guarantee payment of your claim. Patients are responsible for the payment of any deductible and any co-insurance amounts at the time of service. Our office staff and billing company are always available and will be glad to answer any questions.

For your convenience, payment may be made by cash, check, MasterCard, Visa, or Discover. Payment is required at the time services are rendered. In situations requiring special financial considerations, alternative payment plans may be arranged. Please feel free to discuss our fee schedule with our Administrator.

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
